Pre admission questionnaire

Please check the 'Questionnaire' section in your MySpire account. If this is blank please contact the pre-assessment team at the Spire hospital where your planned admission is scheduled if you were asked
to complete it online.

Once submitted, the completed questionnaire is passed to the pre-assessment nurses to review. Please call the pre-assessment team at the Spire hospital where your planned admission is.

The questionnaire should be completed and submitted
online, which will then be reviewed by the pre-assessment team at
your hospital. A printed copy of the questionnaire is not required by the hospital team.

Your questionnaire is automatically saved every time you select 'Next' to move onto the next section. You can log out and resume the questionnaire at a later stage, if necessary.
